OS-HELP Loan Application
Apply for an OS-HELP Loan below.
Along with this form you will need to submit the below documentation. If you do not yet have these, please make sure they're organised then come back and complete this form.

1. Evidence of Australian Citizenship, Permanent Humanitarian Visa holder or New Zealand Citizenship with Special Category Visa (SCV).


       Examples of acceptable forms of documentation include:
       Australian citizen
         - A copy of current Australian passport
         - Australian birth certificate, OR
         - Australian citizenship certificate

        Australian Permanent Humanitarian visa holder
          - A copy of current passport AND visa

        NZ citizen with SCV
          - A copy of current New Zealand passport AND New Zealand Special Category visa (SCV)
          - If you are in the process of renewing your passport, please attach a copy of your old passport AND a copy of your confirmation of passport renewal

2. Completed Mobility Coordinator Form
      This form can be downloaded here and must be signed by the Curtin staff member who is organising and/or signing off on your overseas study program. Staff signing off must be from your faculty/school or the Study Abroad Office.

3. Proof of Program - either a copy of your acceptance or enrolment letter from your host institution, OR a copy of your study tour itinerary.
